What is Ohio’s PASSPORT program?
PASSPORT is an Ohio Medicaid waiver program that helps older adults get care at home instead of moving to a nursing home. It is run by the Ohio Department of Aging through local Area Agencies on Aging.
Who may qualify
In general, a person may be eligible for PASSPORT if they:
- Are 60 or older
- Live in Ohio
- Need the level of care usually provided in a nursing home
- Meet Ohio Medicaid’s financial eligibility rules
Your local Area Agency on Aging makes the final decision through an in-home assessment.
What services PASSPORT can cover
Each person gets a care plan built with a PASSPORT care manager. Depending on that plan, services can include personal care (help with bathing, dressing and grooming), homemaker services (light housekeeping, laundry, meals and errands), and other supports that help someone stay safely at home.
