Choosing a provider for DD waiver services
If you or a family member receives an Ohio DODD waiver (Individual Options, Level One or SELF), you get to choose the agency that provides your services. These questions can help you and your SSA pick the right fit.
Questions to ask an agency
- Which waivers do you accept? Make sure the agency is certified for your waiver and the services in your plan.
- How quickly can you start? Ask how long staffing usually takes after a referral.
- Will I have consistent staff? Seeing the same Direct Support Professionals builds trust and keeps routines steady.
- How are staff trained and screened? Ask about background checks and required DODD training.
- How do you communicate with my SSA and team? Good agencies share updates and respond quickly.
- Can staff help in the community? Ask whether staff can escort you to appointments and activities, and how transportation works.
- Who do I call with a problem? You should have a clear office contact and know when the office is open.
